Christian Service Permission Forms

Permission forms. 

 

Agencies – A list of pre-approved agencies can be found in the PDF below, and Campus Ministry Office. All service opportunities if not on the listed pre-approved agencies, must fall within the Rubric or be Pre-Approved by the Christian Service Coordinator.

Service Verification Form – Complete the Service Verification Form during each service experience, by printing the PDF below. Then upload the completed service verification forms to the student's Google Classroom for Christian Christian Service. Remember that service hours are due two weeks before the start of scheduled final exams each semester. 

Due Dates:

  • Everyone on December 2, 2024 by 3:30PM
  • Seniors hours are due May 2, 2025, by 3:30 PM
  • Underclassmen on May 12, 2025, by 3:30 PM

 

 

 Canned Food Drive --TBD

*12 cans = 1 hour

Print and Complete the Service Hour Verification Form and bring it to Mentoring Period teacher to be signed at the time of donation only (1 form for every 12 cans). Max two hours

Donations are accepted in room 309

Cans must be in good condition (no dents) and have an expiration date of more than 6 months

from the date of acceptance.

Toy Drive - TBD

One hour of service for any new toy with the value of ten dollars. Max two hours. Toys can be dropped off to room 309

Hours will apply to the spring semester

Midnight Mission – September    In order to participate in this service opportunity please print the following PDF file below: 1-Midnight Mission Student Permission Form 9/20/2024. Complete all forms and turn in to Mr. McDorman in room 309.  Space is limited and participants will be accepted based on completed forms received on a first come, first served basis.

Special Olympics – TBD (Closed) In order to participate in this service opportunity please print the following PDF file below: TBA. Complete and submit all forms to Mr. McDorman in room 309 as soon as possible. Mr. McDorman will confirm your Registration with your assigned number.  You are NOT REGISTERED unless you receive your number from Mr. McDorman.  Space is limited and participants will be accepted based on completed forms received on a first come, first served basis. If you are registering from this point on you will need to provide your own transportation to and from the event and bring all original completed copies of the required forms to the event.

PROCEDURES - Google Classroom:

 

Students each semester will utilize Google Classroom to turn in their Christian Service Hours.

 

Each student will be given a code that will correspond to their grade level. Please, sign up for your respective class:

- Please email Mr. McDorman ([email protected]) for Google Class code if you did not receive it in your school email.

 

Then, students will take a digital photo or scan in their forms. The forms must be attached to the "Christian Service Assignment".  

 

Students must handwrite forms. No typed reflections will be accepted.

 

Finally, please ensure that completed Christian Service Hour Forms are uploaded to the proper Google Classroom, in the respective assignment, by the respective due date.

Midnight Mission Permission Forms

Addition Form for Midnight Mission:

Please Complete the following:

  1. Submit your Volunteer Waiver 48 hours before your service date. You can do so from any device by clicking this link: igfn.us/f/sk2/nPlease fill one out even if you have done so already.
  2. Submit the full names of everyone in your party 48 hours before your service. Please note that we will assume that you are cancelling if we do not receive the list on time.
  3. Please arrive 15 minutes early to do a COVID-19 rapid test.
