Request Official Transcript or Diploma

Official Transcript Request:

To request your official transcripts - please send an email to: [email protected]

Complete the Transcript Request Form.

1. Include your full name
2. Social Security Number is not necessary
3. Indicate what institution(s) you wish the transcript mailed to
4. Indicate if you will be picking up the transcript yourself
5. Official transcripts are $5 each - will not be processed until payment is received

Duplicate Diploma Request:

 To request your duplicate diploma - please send an email to: [email protected]

1. Send your name, DOB, year of graduation
2. How you wish your name on the duplicate diploma
3. Duplicate diplomas will not be processed until payment is received
4. Indicate when you will pick up the duplicate diploma
5. Duplicate diploma is $25 - must be pay online.

All other inquires regarding an official transcript or duplicate diploma must be sent via email and must include student's name.  No verbal request for transcripts or duplicate diploma.
